Quarterly Report Requirements

Each quarter, we require our fiscal sponsees to turn in a report about the previous quarter. Please see the template below for the kind of information we require.

Narrative Report

In your report, please only include information regarding your organization’s charitable activities.

Answer the following questions. All questions are required for a complete narrative report.

  1. What activities/operations were conducted this quarter/month?
  2. Where were the activities/operations conducted?
  3. Roughly, what percentage of total time is allocated for each of the activities/operations?
  4. Did you earn any revenue or receive any donations other than the grant disbursements under this Fiscal Sponsorship Agreement? If so, how much and from what sources?
  5. How do the activities listed in 1a further your charitable mission?
  6. Provide any other information relevant to your activities/operations not addressed above.

 

If the information contained in these reports is insufficient, then Sponsor may require and Sponsee shall provide additional reporting. These reports should be signed and dated by the preparer. Please return both reports quarterly to the Sponsor.
